Use this form to capture a customer name, phone, email, project notes/details and upload files. Add information to other fields, the more information the better, but there are only three required fields to submit the form.

You add “Total SqFt” AND “Cost/SqFt” and the form calculates “Quote for SqFt.” Use “Additional Cost(s)” to add a flat cost to the TOTAL ESTIMATE. There are descriptions for the additional costs. 

If “Send to Customer” is “YES” customer is emailed a project estimate. YOU MUST INCLUDE AN EMAIL ADDRESS IN THE “EMAIL” FIELD TO SEND CUSTOMER EMAIL ESTIMATE. Change the name of the “Quoted by” field to add your name to the signature of the customer email estimate.
